Investigation

The Theemis team has extensive expertise conducting national inquiries, wider system reviews of incidents and local serious incident investigations. This expertise includes regulatory investigations, domestic homicide reviews, deaths in mental health settings, national and local safety investigations and national inquiries.

Service reviews

At Theemis, we are experts in undertaking service reviews at a local and system-wide level. We work collaboratively with our clients to understand their requirements in granular detail. Our expertise lies in our ability to design reviews using evidence-based inquiry methods to collect and analyse information about complex services and systems.

We synthesise information in a way that assists our clients to understand how their system operates from different perspectives. These evidence-based methods explore cultural influences, alongside demand and supply requirements and place the experience of those using the service at the centre of our work.

We have significant expertise writing and analysing organisational policies to support our clients delivering critical services to their populations.

Applied research

Theemis is experienced in developing mixed method approaches to address local system problems. Using these methods, we assist our clients to understand their service or system and the impact different influences have upon them. Our team has contributed to the national knowledge base about safety across a range of different services through investigations, reviews and publications in peer-reviewed journals.

Training

Theemis can offer training on all the processes required within an investigation, including investigative interviewing, family engagement, analysis methods and report writing. The training can be co-designed with clients to ensure it is tailored to specific requirements.

We can provide education around service reviews and human factors approaches to understand factors that contribute to how systems succeed and fail.

Governance reviews

Theemis is experienced in understanding complex governance systems and identifying where processes can be simplified to improve the flow of communication. Theemis has experience of developing new governance processes and training staff to ensure any new system is embedded and functioning as intended. Our expertise in this area includes reviews of boards and leadership and making recommendations for improvement.

Serious incidents and complaints

We have expertise in developing new serious incident and complaint handling processes. This includes the development of all associated policies and procedures and the delivery of training for staff. We adopt a systems approach to maximise learning from information gathered from serious incidents and complaints. Where systems are not operating as intended, Theemis has experience in reviewing and recommending improvements to the process.
